Ohio licenses assisted living under a single RCF license. Ohio issues one Residential Care Facility (RCF) license covering nearly all assisted living and memory care, under ORC 3721 and OAC 3701-16. There is no evacuation-based tiering and no separate small-home license type. Read the full explanation →

What to ask on a tour in Cleveland

Marketing directors are good at their jobs. These are the questions that actually surface differences between communities:

  1. Can I see your current RCF license and most recent ODH inspection report? Ohio uses one license type, so the license itself will not tell you as much as the inspection history will.
  2. If you operate a special care unit, can I see the written disclosure? Required under OAC 3701-16-07(F)(5) — it is not the license, and a genuine special care unit will produce it without hesitation.
  3. What is the all-in monthly cost at my parent's current care level? Ask for the care-level assessment tool in writing and where your parent would score today.
  4. How often have rates increased in the last three years, and by how much?
  5. What is the overnight staffing ratio? Not the daytime ratio — overnight is where families get surprised.
  6. What is caregiver turnover here? A community that tracks it and will tell you is a different kind of operator than one that deflects.
  7. Do you hold an Assisted Living Waiver provider agreement, and how many current residents use it? Many Cleveland-area communities do not participate, and waiver capacity is limited even where they do.
  8. What happens if my parent's needs exceed the 120-day skilled nursing cap? Ask specifically what the transition plan looks like and whether it means changing buildings.

Go back unannounced at a different time of day before you decide. A mid-morning tour and a 7 p.m. drop-in can look like two different buildings.

Questions Cleveland families ask

What's the phone number to report suspected elder abuse in Cuyahoga County?

Cuyahoga County's Division of Senior and Adult Services runs a 24-hour reporting line at 216-420-6700 for abuse, neglect, self-neglect, and financial exploitation of adults 60 and older living in the community. Call 911 first if someone faces immediate risk of death or serious harm.

Is reporting abuse different for a facility resident versus someone living at home in Ohio?

Yes. Abuse or neglect inside a licensed nursing home or residential care facility should be reported to the Ohio Department of Health complaint line (1-800-342-0553), since facility-based care is an ODH licensing matter, while abuse or neglect of someone living at home goes to county Adult Protective Services, 216-420-6700 in Cuyahoga County, or the relevant county JFS/DJFS agency elsewhere.

What happens after someone files an Adult Protective Services report in Ohio?

Once a county APS agency, such as Cuyahoga County's Division of Senior and Adult Services, receives a report, caseworkers assess the situation, which may include a home visit, interviews, and coordination with law enforcement or medical providers if warranted. Outcomes range from connecting the individual with support services to, in serious cases, pursuing legal intervention such as guardianship; APS acts to protect the adult's safety while respecting their right to make their own decisions where they have capacity to do so.
